This plan assists them in recognizing signs of relapse and taking full responsibility for their health and well-being. The process involves deepening self-awareness, understanding and reflecting on past experiences, developing skills to recognize signs of relapse, and ultimately creating an actionable plan to prevent and manage potential relapses.
