With the advent of industrialization and inactive lifestyle, the use of fast food has become a trend in the modern world specially burger, french fries, pizzas confectionery like pastries, cakes that are rich with unsaturated trans fatty acid. This became one of the etiology for insulin resistance and type 2 Diabetes. This study was conducted in four ethnic groups of Quetta city Balochistan, Pakistan namely Baloch, Pashtune, Hazara and Urdu speaking. It shows that both men and women are at high risk for the diseases and prevalence of this health issue is possible by changes in lifestyle and eating patterns; moreover, abrupt use of traditional Pakistani food that is rich with trans fatty acid because of its taste is another contributory factor towards the onset of this disease, because of all these factors the incidents of diabetes increasing while the management of this disease is out of control along with its risk factors. Currently Pakistan stands at number 7th in the list of countries in terms of prevalence of DM and if the current situation persists, it is forecasted to move to 4th place. This alarming situation is a massive challenge for Pakistani health care professionals.
